Only manual wheelchairs can be used with the Freedom Trax Device. If user arrives with a power wheelchair then a loaner manual chair will be provided for the user to go on the Freedom Trax Device.

Clearance under lowest part of the manual wheelchair to the ground (inches).
Will the wheelchair user be accompanied on the field trip by an aid/parent/spouse/etc? If yes please provide name of the person joining.
Does the manual wheelchair have any modifications? (Shocks on casters, vehicle mounting bracket, all terrain wheels, solid casters, caster extensions, modified footplate etc. We will call or email you to discuss how this may affect your use of the FreedomTrax)
